MEMBERSHIP agreement

1. MEMBERSHIP BENEFITS

While your Everyday Beauty Membership is active and in good standing, you receive:

• 10% off eligible skincare products
• 15% off eligible facials and laser services
• $1.00 off per unit of Botox®
• $0.33 off per unit of Dysport®
• $50 off each eligible syringe or vial of injectable treatment

Membership discounts generally apply to eligible individual, a la carte services and products unless otherwise stated.

2. MONTHLY MEMBERSHIP FEE & AUTOMATIC BILLING

The Everyday Beauty Membership fee is $175 per month.

Your first membership payment will be charged when you enroll. By enrolling, you authorize Everyday Beauty to automatically charge the payment method you provide each month thereafter during your membership.

The membership fee provides access to membership benefits and preferred member pricing. The $175 monthly membership fee accumulates as account credit and can be applied as a dollar for dollar credit toward treatments, services, or products at Everyday Beauty.

Membership payments that have already been processed are final and non refundable, except where otherwise required by law.

3. 90 DAY MINIMUM COMMITMENT

The Everyday Beauty Membership has an initial minimum commitment of 90 days.

During the initial 90 day period, you agree to make three monthly membership payments of $175, for a total minimum membership commitment of $525.

You may request cancellation during the initial 90 day period, however you remain responsible for membership payments due during the 90 day minimum commitment.

If you submit a cancellation request during the initial commitment, Everyday Beauty will schedule the membership to end when the initial 90 day commitment has been satisfied. You will not be required to submit another cancellation request.

After the initial 90 day period has been completed, the membership automatically continues on a month to month basis until canceled.

4. EASY CANCELLATION

Everyday Beauty intends to make canceling your membership simple and accessible.

If you enrolled in your membership online, you may also cancel your membership online without being required to call, visit the clinic, or speak with a representative. Online cancellation is available at everydaybeautysalon.com.

If you enrolled in person, you may cancel online at everydaybeautysalon.com or by calling (480) 608-9106. You may also request cancellation in person if you choose, but an in person visit will not be required to cancel.

Everyday Beauty will not require unnecessary additional steps, mandatory retention calls, or an in person visit as a condition of canceling. Reasonable identity verification may be required to protect your account.

If cancellation is requested during the initial 90 day commitment, the membership will be scheduled to end once the initial commitment has been completed.

After completion of the initial 90 day commitment, a cancellation request will stop future recurring membership charges. Any membership payment already processed before cancellation is non refundable except where required by law.

Membership benefits associated with a paid membership period will generally remain available through the end of that paid period.

5. PACKAGES & BEST AVAILABLE VALUE

Membership discounts do not apply to service packages.

Everyday Beauty packages are already priced at discounted rates to provide savings when multiple treatments or services are purchased together. Because packages already include discounted pricing, an additional membership discount cannot be applied to or stacked on top of package pricing.

However, being a member should not prevent you from receiving the best available value. When applicable, our team will help compare your membership price for individual services with available package pricing for those services.

If a package provides a better overall value for your recommended treatment plan, we may recommend purchasing the package instead of using your membership discount on individual services. Our goal is to help you choose the eligible pricing option that provides you with the better value.

Package pricing and membership discounts cannot be combined or stacked.

6. PROMOTIONS & OTHER DISCOUNTS

Unless Everyday Beauty specifically states otherwise, membership discounts cannot be combined or stacked with:

• Packages
• Promotional pricing
• Special offers
• Coupons
• Event pricing
• Promotional treatment bundles
• Other discounts

If multiple eligible offers are available for the same service, Everyday Beauty may help you determine which eligible pricing option provides the better value.

7. INDIVIDUAL & NON TRANSFERABLE MEMBERSHIP

Your Everyday Beauty Membership belongs only to you. Membership benefits, discounts, preferred pricing, and other membership perks:

• Cannot be shared with friends or family
• Cannot be transferred to another person
• Cannot be sold
• Cannot be gifted to another person
• Cannot be redeemed for cash
• Cannot be used to purchase services or products for another individual at the member rate

Each individual who wishes to receive membership benefits must maintain their own active Everyday Beauty Membership.

8. ACTIVE MEMBERSHIP REQUIRED

Membership benefits are available only while your membership is active and in good standing. A valid payment method must remain on file.

If a monthly payment is declined, membership benefits may be suspended until the account is brought current. Everyday Beauty may notify you of the failed payment and may attempt to process the authorized payment method again.

Membership discounts cannot be applied retroactively to purchases made before enrollment or while the membership was inactive.

9. MEDICAL ELIGIBILITY

Membership does not guarantee eligibility for any medical or aesthetic treatment. Treatments may require:

• Medical consultation
• Medical history review
• Provider evaluation
• Treatment specific consent
• Medical clearance or approval

Everyday Beauty and its licensed providers reserve the right to recommend against, postpone, modify, or decline any treatment when medically appropriate. Membership enrollment does not replace treatment specific medical consent forms or other required documentation.

10. APPOINTMENTS & CANCELLATION POLICIES

Membership does not guarantee appointment availability.

All appointments remain subject to Everyday Beauty's standard appointment, cancellation, rescheduling, late arrival, and no show policies.

Appointment cancellation or no show fees, when applicable, are separate from membership fees.

11. CHANGES TO MEMBERSHIP TERMS

Everyday Beauty may periodically modify membership benefits, eligible services, exclusions, or future membership pricing.

No increase to your monthly membership price or other material change will apply during your initial 90 day commitment without your agreement.

For material changes that affect an existing month to month membership, Everyday Beauty will provide reasonable advance notice before the change takes effect. Changes will not be applied retroactively.

12. TERMINATION BY EVERYDAY BEAUTY

Everyday Beauty may suspend or terminate a membership for reasons including:

• Failure to pay membership fees
• Fraudulent activity
• Sharing or attempted sharing of membership benefits
• Abuse or misuse of membership benefits
• Inappropriate or threatening conduct
• Violation of Everyday Beauty policies

Termination by Everyday Beauty does not limit any rights a client may have under applicable law.

RECURRING PAYMENT AUTHORIZATION & EXPRESS CONSENT

Please read before agreeing. I understand that:

• My Everyday Beauty Membership costs $175 per month.
• I am committing to an initial minimum period of 90 days and three monthly payments totaling $525.
• My payment method will be automatically charged $175 each month.
• After the initial 90 day period, my membership automatically continues month to month until I cancel.
• I may request cancellation at any time. If I cancel during my initial 90 day commitment, my cancellation will automatically take effect after my minimum commitment has been satisfied, and I will not need to cancel again.
• After my initial 90 day commitment, I may cancel before a future recurring charge using the cancellation methods described in this Agreement.
• Membership payments already processed are non refundable except where required by law.

Required member consent: I have read and understand the recurring billing, 90 day minimum commitment, automatic renewal, and cancellation terms above. I expressly authorize Everyday Beauty to charge my payment method $175 per month as described in this Agreement.

MEMBERSHIP ACKNOWLEDGMENT

By enrolling, I acknowledge that:

• I received and had an opportunity to review the membership terms before enrolling.
• I understand the membership price and recurring billing schedule.
• I understand the 90 day minimum commitment.
• I understand that the membership automatically continues month to month after the initial commitment until canceled.
• I understand how to cancel my membership.
• I understand that processed membership payments are non refundable except where required by law.
• I understand that membership discounts do not apply to packages.
• I understand that package and membership discounts cannot be stacked.
• I understand that Everyday Beauty may help me determine whether membership pricing or package pricing provides the better eligible value.
• I understand that membership benefits belong only to me and cannot be shared or transferred.
