The mechanical difference
Semaglutide mimics GLP-1, a hormone your gut releases after eating that signals fullness, slows stomach emptying, and steadies blood sugar. Tirzepatide acts on GLP-1 as well as GIP, a second gut hormone involved in how your body handles fat and glucose. Acting on two pathways rather than one is why Tirzepatide has generally produced greater average weight loss in clinical trials.
What that means for results
On average, Tirzepatide outperforms Semaglutide on total weight lost. Averages are not promises, though. Plenty of clients reach their goal comfortably on Semaglutide, and individual response varies more than the headline numbers suggest. Your starting weight, dose you tolerate, and how consistently you support the medication with protein and resistance training all move your outcome more than the choice between the two.
Side effects
Both share the same profile, nausea, reduced appetite, constipation or diarrhea, and fatigue, strongest in the days after a dose increase and easing as your body adapts. Some clients find one more tolerable than the other, and switching is possible if side effects are limiting your progress. Starting at a low dose and titrating slowly is the single most effective way to keep side effects manageable, regardless of which medication you are on.
Cost and practicalities
Tirzepatide typically costs more. Both are weekly self administered subcutaneous injections using a very small needle, and both require the same monthly check ins and dose adjustments.
