How Long Does Botox Last? A Realistic Timeline

When results appear, how long they hold, and the habits that make a treatment wear off sooner than it should.

The short answer

Three to four months is the honest range for most people. Some clients hold results closer to five months, others start seeing movement return around week ten. That spread is normal and it comes down to your metabolism, how strong the treated muscles are, how many units were used, and how consistently you maintain your treatments.

The first two weeks

Nothing happens immediately, and that surprises people. Wrinkle relaxers work by blocking the signal between nerve and muscle, which takes a few days to take hold. You will usually notice softening at day three to five, real change by day seven, and your final result at the two week mark. This is why we schedule a follow up at two weeks rather than judging the outcome on day three, and why we never add more units before then.

Why it wears off sooner for some people

A few factors shorten the interval reliably. Very expressive faces and strong muscles metabolize the effect faster. Heavy endurance exercise appears to speed it up for some clients. Underdosing is the most common culprit, if the starting dose was conservative, the result was always going to fade earlier, and that is a fair tradeoff on a first appointment. Stress, illness, and smoking are frequently blamed, with less consistent evidence behind them.

Maintenance actually helps

Clients who stay on a consistent schedule often find their intervals stretching over time. When a muscle is repeatedly kept from contracting, it gradually weakens, and existing lines have a chance to soften rather than deepening with every expression. Waiting until every bit of movement is back before rebooking means starting over each time. Treating just before the result fully wears off tends to hold better with the same number of units.

When to book your next appointment

Watch for lines starting to reappear when you make an expression, that is the signal, usually somewhere between month three and month four. Booking a couple of weeks ahead of that point is ideal. If you have an event coming up, plan your appointment at least three weeks out so your result has fully settled and any pinpoint bruising has resolved.
